Information Clause On Processing Personal Data
For Participants Of Polonium Mentoring Programme

In connection with participation in a mentoring programme organised by the Polonium / Polonium Foundation (details on https://poloniumfoundation.org/poloniummentoring) I accept the content and rules of personal data processing:
1. The administrator of personal data is the Polonium Foundation with the legal address of Pl. Bankowy 2, 00-095 Warsaw, Poland, registered by the District Court for the Capital City of Warsaw in Warsaw (KRS 0000621890, NIP: 7010584408, Regon: 364666620), e-mail address: contact@poloniumfoundation.org (hereinafter referred to as: Foundation or Administrator).
2 The Administrator has appointed a Data Protection Officer. Any questions related to the processing of personal data by the Foundation can be sent to the e-mail address: iod@poloniumfoundation.org or to the address indicated in point 1.
3. Personal data will be processed on the basis of:
· Article 6(1)(a) GDPR Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) - consent to the processing of personal data;
Article 6(1)(b) GDPR, i.e. their processing is necessary for the performance of the contract of participation in the mentoring programme;
Article 6(1)(c) GDPR to issue a certificate confirming participation in the mentoring programme;
Article 6(1)(f) GDPR to make a possible claim or defence against a claim.
4. Personal data will be processed to enable participation in the mentoring programme. If you give your consent, your data will also be processed for marketing and/or commercial purposes, e.g. to provide information about planned further editions of the mentoring programme.
5. The recipient of personal data will be the authorised employees/cooperators of the Administrator, who must have access to them in the performance of their duties related to the implementation of the event.
6. Personal data will not be transferred to third countries, i.e. outside the European Economic Area.
7. Personal data will be processed by the Administrator until revocation of consent. Consent can be revoked by sending an e-mail to: iod@poloniumfoundation.org.
8. To the extent and under the rules arising from the GDPR, the data subject has the right of access to the content of their personal data and the right to rectify, delete, limit the processing, the right to transfer, the right to object.
9. The data subject has the right to lodge a complaint with the supervisory body, i.e. the President of the Office for the Protection of Personal Data, if they consider that the processing of personal data violates the provisions of the GDPR.
10. The provision of personal data is not obligatory, but it is necessary to take part in the mentoring programme.
11. Personal data shall not be processed for the purpose of automated decision making, including profiling.

Klauzula Informacyjna O Przetwarzaniu Danych Osobowych
Dla Uczestników Polonium Mentoring Programme
W związku z udziałem w programie mentoringowym organizowanych przez Fundacja Polonium / Polonium Foundation (szczegóły na stronie https://poloniumfoundation.org/poloniummentoring) akceptuję treść i zasady przetwarzania danych osobowych:
1. Administratorem danych osobowych jest Fundacja Polonium / Polonium Foundation z siedzibą Pl. Bankowy 2, 00-095 Warszawa, Polska/Poland, wpisana przez Sąd Rejonowy dla m. st. Warszawy w Warszawie, XII Wydział Gospodarczy Krajowego Rejestru Sądowego pod numerem KRS 0000621890, NIP: 7010584408, Regon: 364666620 adres e-mail: contact@poloniumfoundation.org (dalej jako: Fundacja lub Administrator).
2. Administrator powołał Inspektora Ochrony Danych Osobowych. Wszelkie pytania związane z przetwarzaniem przez Fundację danych osobowych można kierować na adres email: iod@poloniumfoundation.org lub na adres wskazany w pkt. 1 powyżej.
3. Dane osobowe przetwarzane będą na podstawie:
art. 6 ust. 1 pkt. a) RODO - Rozporządzenie Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/we (ogólne rozporządzenie o ochronie danych) - zgoda na przetwarzanie danych osobowych;
art. 6 ust. 1 pkt. b) RODO, tj. ich przetwarzanie jest niezbędne do wykonania umowy uczestnictwa w programie mentoringowym;
art. 6 ust. 1 lit. c RODO wystawienia zaświadczeń o udziale w programie mentoringowym;
art. 6 ust. 1 lit. f RODO w celu ewentualnego dochodzenia roszczeń lub obrony przed roszczeniami.
4. Dane osobowe będą przetwarzane w celu umożliwienia udziału w programie mentoringowym. W przypadku wyrażenia przez Panią/Pana zgody dane będą również przetwarzane na cele marketingowe i/lub handlowe np. dla potrzeb przekazania informacji o planowanych przyszłych edycjach programu mentorinowego.
5. Odbiorcą danych osobowych będą upoważnieni pracownicy/współpracownicy Administratora, którzy w ramach wykonywania swoich obowiązków związanych z realizacją wydarzenia muszą posiadać do nich dostęp.
6. Dane osobowe nie będą przekazywane do państw trzecich, czyli poza Europejski Obszar Gospodarczy.
7. Dane osobowe będą przez Administratora przetwarzane do odwołania zgody. Zgodę można odwołać poprzez wysłanie wiadomości e-mail na adres: iod@poloniumfoundation.org.
8. W zakresie oraz na zasadach wynikających z RODO, osoba, której dane dotyczą, posiada prawo dostępu do treści swoich danych osobowych oraz prawo ich sprostowania, usunięcia, ograniczenia przetwarzania, prawo do przenoszenia, prawo do wniesienia sprzeciwu.
9. Osoba, której dane dotyczą, ma prawo do wniesienia skargi do organu nadzorczego, to jest Prezesa Urzędu Ochrony Danych Osobowych, gdy uzna, iż przetwarzanie danych osobowych narusza przepisy RODO.
10. Podanie danych osobowych nie jest obowiązkowe, jednak jest niezbędne wzięcia udziału w programie mentoringowym.
11. Dane osobowe nie będą przetwarzane w celu zautomatyzowanego podejmowania decyzji, w tym również w formie profilowania.
